The pass is valid for these monuments:

Tower of St. Nicholas Church - Vez chramu sv. Mikulase

It is valid for 3 days from the date you visit your first monument in the high season.
Price: 400 CZK/person or 200 CZK/person (discount for kids, students, pensioners)
You can buy your 10 Monuments Pass directly at the respective monuments or at Prague Information Service Tourist Information Centers (Prague Ruzyne Airport, Main Railway Station – station hall, Old Town Hall, Rytirska 31, Lesser Town Bridge Tower).
